Een behandeling met avutometinib plus defactinib bij patiënten met laaggradig sereus ovariumcacinoom liet hoge responspercentages zien, ongeacht de KRAS-mutatiestatus. Dit bleek uit de fase 2-RAMP 201-studie, waarvan dr. Susana Banerjee (Londen, Verenigd Koninkrijk) de resultaten presenteerde tijdens het ESGO 2023 Congress. “Het bevestigde objectieve responspercentage was 45% in de hele studiepopulatie en 60% bij de patiënten met een KRAS-mutatie.”
Laaggradig sereus ovariumcacinoom (LGSOC) is een zeldzame vorm van kanker die minder dan 10% van de ovariumcarcinomen uitmaakt, zei Susana Banerjee. “Met de huidige behandelopties voor gerecidiveerd LGSOC zijn de responspercentages laag, variërend van 0% tot 26%. Maar we weten ook dat de RAS/MAPK-signaaltransductieroute een belangrijke rol speelt bij de pathogenese van deze tumoren.”
Avutometinib is een RAF/MEK-remmer die zowel MEK-kinaseactiviteit als de compenserende reactivering van MEK door RAF inhibeert. Activering van FAK is een mogelijk resistentiemechanisme tegen remming van RAF/MEK. Door defactinib, een selectieve remmer van FAK, te combineren met avutometinib kan dit resistentiemechanisme mogelijk overwonnen worden, legde Banerjee uit. Een fase 1-studie liet al hoge responspercentages zien van een behandeling met avutometinib/defactinib, wat leidde tot een Breakthrough Therapy Designation van de Amerikaanse Food and Drug Administration en de rationale vormde voor de RAMP 201-studie.1
Optimaal behandelregime
De RAMP 201 is een fase 2-studie naar avutometinib alleen of in combinatie met defactinib bij patiënten met gerecidiveerd LGSOC.2 Zowel patiënten met een KRAS-mutatie als KRAS-wildtype werden geïncludeerd. De patiënten moesten eerder platinumbevattende chemotherapie hebben gehad en een eerdere behandeling met een MEK-remmer was toegestaan.
Het belangrijkste doel van de studie was het bepalen van het optimale behandelregime en de optimale patiëntenpopulatie voor dit regime. Dit werd gedaan aan de hand van de responspercentages, waarbij het gevonden verschil tussen de groepen minimaal 15% moest zijn.
Banerjee presenteerde de resultaten wat betreft werkzaamheid voor 33 patiënten die behandeld waren met alleen avutometinib en 31 patiënten die de combinatie hadden ontvangen. De veiligheidsdata werden gepresenteerd voor de totale studiepopulatie (n=151).
Afname tumorgrootte
Het objectieve responspercentage met de monotherapie was 10% versus 45% met de combinatie. Dit was ongeacht de KRAS-mutatiestatus. Bij patiënten met een KRAS-mutatie was de ORR 13% met alleen avutometinib en 60% met avutometinib/defactinib. “We zagen daarnaast responsen bij drie van de vier patiënten die eerder behandeld waren met een MEK-remmer.”
De mediane tijd tot respons was 7,3 maanden met monotherapie en 5,5 maanden met de combinatie. De duur van de respons en progressievrije overleving waren nog niet behaald ten tijde van deze analyse. In totaal liet 90% van de patiënten die alleen avutometinib ontvingen en 87% van de patiënten die de combinatie ontvingen een afname in tumorgrootte zien. De respons was dieper met de combinatie, aldus Banerjee.
Het grootste deel van de behandelingsgerelateerde bijwerkingen betrof graad 1 of 2. De patiënten rapporteerden met name misselijkheid, diarree en een verhoging van de creatinekinasewaarden in het bloed.
“Met deze fase 2-studie hebben we het regime geïdentificeerd waarmee verder onderzoek gedaan gaat worden”, zei Banerjee. “Inmiddels is een gerandomiseerde fase 3-studie opgezet waarin de combinatie van avutometinib/defactinib versus de standaardbehandeling onderzocht gaat worden bij gerecidiveerd LGSOC: de RAMP 301-studie.”
